Dear Patient,
It is with mixed emotions that I am announcing my retirement from the active practice of Obstetrics and Gynecology, effective May 15, 2024. It has been a great pleasure providing care for you over the past 33 years, and it is not easy for me to give this up. I will miss the relationships we have developed over time.
Fortunately, I know you are being left in good hands should you choose to continue your care here at Northwest Women’s Center. My current associates (physicians, nurse practitioner, or certified nurse midwife) will be available to meet your needs and the practice is planning to bring on two new physicians this fall. Of course, you may seek medical care from another doctor or practice if you like.
If you choose to seek care elsewhere, I recommend that you look for a new physician at your convenience.
Northwest Women’s Center will remain custodians of your health care records. These records are confidential and a copy of them will be released to you or another person you designate only with your permission. If you plan to continue care here, your records will be available in our system for your next visit. If you choose to see a physician from another practice, please complete the Release of Private Health Information request located on the portal (Health Records) or in our office and return to Northwest Women’s Center so we may transfer your records to your new physician. Until then, your records will remain on file here.
